Mitchell Care Facilities — $1.4M

In Mecklenberg County, Mitchell Care Facilities has provided in-home and online mental health care to individuals for nearly 10 years. In partnership with area hospitals and ambulatory providers, they have focused on using existing online infrastructures to digitize medical records and ensure that time is wasted on bureaucracy. As a result they are able to provide same-day care to those at the most risk. With funding from The Duke Endowment, the Mitchell model will be implemented in 15 counties across North Carolina and South Carolina and will serve an estimated 20,000 individuals.

Buff Care Facilities — $1.4M

Buff Care Facilities has provided in-home and online mental health care to individuals for nearly 10 years. With the emergence of COVID-19, we saw workforce shortages increase the treatment gap, and a fragmented service delivery system result in a lack of early identification and intervention, often leading to symptoms being ignored until they became acute.
